At SPORT P.C., we are dedicated to providing excellent medical care to each of our patients. In addition, our billing department is dedicated to providing prompt and accurate billing to your insurance carriers.

We ask your help by providing us with the most accurate and up to date information so that we may minimize the number of billing errors and confusion.

In the ever changing insurance industry it is imperative that we have the following information to allow for accurate and timely billing.

We must be provided with the following information.

• All current demographic information. (Please be sure to completely and accurately fill out the applicable forms provided on this website under the "Your First Visit" tab.)


• If the patient is a minor, we will need all of the above information for both parents and the responsible party (i.e. parent, legal guardian, power of attorney, etc.)


• Current insurance information. We will need to scan your insurance card(s) in addition to a photo ID. Please bring these to your visit. Please refer back to the "Your First Visit" tab to ensure you have obtained all the information needed.


• We now offer our patients the convenience of paying their account balances online through our website. If you would like to pay your bill online, please click the following link: Online Pay
